On-line Scholarship Application

THIS APPLICATION IS FOR STUDENTS WHO HAVE COMPLETED THE SCHOLARSHIP APPLICATION AND HAVE BEEN SELECTED AS A SEMI-FINALIST BY THE SCHOLARSHIP COMMITTEE.   DO NOT BEGIN THIS APPLICATION UNLESS YOU RECEIVED AN EMAIL FROM THE SCHOLARSHIP COORDINATOR.

 

Supplementary Scholarship Application - The Community Foundation for Palm Beach and Martin Counties manages over 100 scholarship funds. By completing the supplementary application, you will be considered for all scholarship funds.  Please remember click the "save and continue button" after each section is completed. You may log out at any time, but remember your password. DO NOT START ANOTHER APPLICATION. You will need it to log back in to continue your application. Questions marked with a red dot are required, and cannot be left blank. Deadline for all applications is 5:00 p.m. March 28, 2012. All applications and requested information become the property of the Community Foundation for Palm Beach and Martin counties Scholarship Fund. All information must be supplied. If any information is incomplete, the application will be rejected. Please be aware of that some questions have a maximum word count. To be eligible for a Community Foundation Scholarship, applicants must have completed and submitted the Free Application for Federal Student Aid (FAFSA). You will be required to submit the resulting Student Aid Report to us if you are selected to interview. If you are attending a private colleges or universities please submit a CSS PROFILE to determine your eligibility for non-government financial aid, such as the institution's own grants, loans and scholarships.

Gather all required information to upload into your application:

 

  • 1040 Tax form- Page 1 & 2 Questions 1 to 77.  Do not send any schedules or additional pages.  Please white-out ALL social security numbers.
  • High School Transcripts, grades 9 to 12.  Include the "Cumulative Summary" page.  The summary page should show GPA (State and District), SAT/ACT scores, total community service hours and class rank. Do not send pages from grades 1 to 8.  If you have taken college courses, please upload most recent college transcript.
  •  SAR and/or CSS report (if applicable).
  •  All acceptance and/or award letters you received from colleges/universities. These letters should show total cost of attendance less any awards, scholarships, grants or loans accepted or offered.
  •  A minimum of two letters of reference.  Choose adults who know you well.  Teachers can comment about your academic skills, but also consider other people who can write about your talents and abilities, like coaches, employers, community and church/synagogue leaders.  Please do not include letters from family members.  Please make sure the letter is signed by the author.
  • LSP essay - please check requirements before submitting.
  • If you are a US citizen, please upload a copy of one of the following:  birth certificate, passport or naturalization papers.
  •  If you are not a citizen, please include a copy of your resident ID card.

 

  We recommend that you work in a Word document for questions that require lengthy answers. Once complete, you can copy and paste the information into the application.
  • DO NOT use formatting such as bold, italics, colors, etc.
  • SAVE your application frequently as you work. You can log out and return to your application in more than one sitting.
  • Write down the deadline - March 28, 2012 at 5:00 p.m.  We recommend starting your application as soon as possible.  PLEASE do not wait until the last minute. MANY students wait until the last minute which can slow down the application system.

To begin your supplementary application, using the Philanthropy Online system, please log on using the same logon and password you used for the scholarship application.
